Taster Request Form
As we develop new recipes, we want feedback on the flavors and designs.
IMPORTANT: If you have any food allergies or intolerances, please do not sign up. Current recipes cannot be guaranteed to be void of allergens.
We only have room for 20 tasters so it will be first come, first serve. Anyone who is not selected will be added to a wait list.
If selected, you promise to:
Complete an initial short survey about your chocolate preferences.
When samples are provided, you will give honest feedback through the designated form on the design, taste, texture, etc.
If you fail to complete the initial survey or two consecutive feedback forms, you may be removed, and a waitlist participant added.
Samples are never guaranteed, and this process may cease and any time.
If you are still interested and promise to provide honest feedback, please submit the form. We will reach out from our email account individually to confirm your submission.
